Privacy policy

Effective date: August 20, 2026. This policy covers this website and the clinic at 115 Simcoe St. South, Oshawa, ON L1H 4G7.

Who is responsible

PhysioCare Wellness & Rehab is a health information custodian under PHIPA. Day-to-day privacy questions can be sent to connect@physiocarewellnessandrehab.com or 905-240-6266. Individual regulated clinicians are also accountable to their Ontario colleges for the charts they keep.

What we collect

Website forms. Appointment and contact forms ask for your name, email, phone number, preferred treatment, preferred clinician or time, and any notes you choose to share. A hidden field is used only to block automated spam.

Clinical records. In the clinic we collect health history, assessment findings, treatment notes, consents, and billing details needed to provide care. That information is personal health information under PHIPA.

Insurance and claims. If you ask us to direct-bill, we collect insurer, policy, MVA, or WSIB details and the minimum clinical information the payer requires.

Website technical data. Our host may log IP address, browser, and pages visited for security. We use Google Ads (gtag) to measure advertising. We do not use this site to build third-party advertising profiles of patients.

How we use it

• To book, confirm, remind, and provide treatment
• To bill you, your insurer, MVA, or WSIB when you authorize it
• To communicate with your physician or other providers when it is needed for your care, or when you ask us to
• To meet legal, college, and professional record-keeping duties
• To improve clinic operations and this website

Form submissions are emailed to the clinic and stored on our hosting account. We do not sell personal information.

Who we share it with

We share personal health information only as PHIPA allows, including:

• Your circle of care, when needed to treat you
• Insurers, MVA adjusters, and WSIB, when you authorize direct billing or a claim
• Our website host, email provider, and (if enabled) SMTP service, solely to operate booking and contact forms
• Regulators, courts, or others when the law requires it

Service providers are expected to use the information only to perform their work for us.

Safeguards and retention

We use administrative, physical, and technical safeguards appropriate to a community clinic — private treatment rooms, access limited to staff who need the record, and password-protected systems. Website enquiries are kept long enough to complete your request and for ordinary administration. Clinical charts are kept for the retention periods required of regulated health professionals in Ontario, then securely destroyed.

Your choices

You may ask to see or correct personal health information we hold, withdraw consent where PHIPA allows, or complain about our privacy practices. Contact us first at connect@physiocarewellnessandrehab.com or 905-240-6266. If we cannot resolve the issue, you may contact the Information and Privacy Commissioner of Ontario at ipc.on.ca.

Some uses (treatment, legal duties, and records we are required to keep) cannot be fully withdrawn. Declining to provide information we need for safe care may limit the treatment we can offer.

This website

This site is hosted in Canada or the United States depending on our hosting account. Do not send detailed medical histories through the public contact form if you prefer to share them in person or by phone.

We use Google Ads (gtag) to measure advertising. Advertising cookies are off until you choose Accept on the site notice. You can decline and still use the site. Google’s own policies describe how they use ads data. You can also control advertising cookies in your browser.

We may update this policy when our practices or the law change. The effective date at the top will be revised when we do.
